Nonocclusive mesenteric ischemia is a disorder seen primarily in elderly patients with cardiac disease and is characterized by progressive intestinal ischemia leading to infarction, sepsis, and death. Dialysis patients are among the highest risk populations for developing this lethal complication. Previous studies have associated atherosclerosis and hemodialysis-induced hypotension as risk factors for nonocclusive mesenteric ischemi development. Clinicians should develop a high index of suspicion for non occlusive mesenteric ischemi in dialysis patients for early diagnosis. Early recognition and resection of gangreneous intestinal segments as earyl as possible may reduce high mortality rates. We presented in this article a 68 -year- old woman who had acute abdominal symptoms following an episode of severe hypotension during hemodialysis developed caecum necrosis spontaneously
