Anocutaneous V-Y Advancement Flap in the Treatment of Anal Fissure

OBJECTIVE:

Lateral internal sphincterotomy is considered the surgical treatment of choice of chronic anal fissure after failure of medical therapy, but it is associated with the risk of anal incontinence, especially in females. The aim of this presented prospective study was to evaluate the efficacy of the treatment with anocutaneous V-Y advancement flap for chronic anal fissure resistant to medical therapy.

METHODS:

Forty patients with chronic anal fissure unresponsive to previous medical treatment, were included in the study. All patients underwent anocutaneous advancement V-Y flap treatment for fissure. Complete healing, relief of symptoms and early and long-tem complications were recorded.

RESULTS:

Complete healing occurred in 39 patients within 30 days. At one year, none of the patients experienced a continence disturbance. Further lateral internal sphincterotomy for recurrent anal fissure was necessary in one patient.

CONCLUSION:

Anocutaneous advancement flap can be used an alternative treatment method in chronic anal fissure treatment.
